I wouldn't be so concerned with it, except that fade could really easily become a nasty slice. I definitely feel like the problem is with my driver head not being aligned, whether that is my wrists being to active during the back swing or my shoulders being to quick on the down swing, I'm not sure exactly, but I feel that my problems are in my club head.

Sorry, my comment above wasn't meant as a dig at you. I've just never in my life heard of anyone trying to analyse a practice swing before. The reason being I've seen many many golfers with lovely, smooth perfect practice swings only to be followed up by something far more sinister and fault ridden as soon as that little white thing gets in the way! :-)
Apologies again for jumping on the camera bandwagon. Unfortunately, in the second two videos, due to the combination of low light, (I'm guessing) low shutter speed / frames per second on the camera you're using it's really hard to see what the club face is doing. Even in the backswing where the club is moving relatively slowly, it's impossible to make out if the face is (as I believe it will be) open.
Is there any way you can gain access to a good quality video camera? Even just borrowing it from a friend for the day. Ideally film during the day when it's clear and bright (but be careful where the sun is!) The behind view (down the line) will show the most important movements in the swing.
I have a number of theories / ideas about your golf swing but I'd really like to get a better look at it first. Also a couple of questions...
You say it's the driver that fades, what happens with the irons? Less fade? The odd pull?
Check and see where your divot points after a shot with an iron.
Could we get a good look at your grip from front on.
What do you 'feel' is happening during your takeaway? Are you trying to do anything in particular with the club, hands, body at this time?
During a 'normal' lesson all this information and more could be found out pretty quickly. Simply analysing one practice swing and a couple of dark range drives without more info is a bit reckless in my honest opinion. That's not meant as a dig at the guys who have commented and offered their help. Some of my thoughts are along the same lines but I will say more a) if you are interested and more importantly if b) we can get some clearer footage.
